The registration process to participate in the Biometric Wellness Screening with Quest Diagnostics will soon be available for those enrolled in Mosaic’s Aetna health plan. You will receive an email notification from Quest Diagnostics the week of April 7th to notify you when the registration portal is available. By participating in the screening, you will be eligible for the lowest monthly premium.  Those not participating will pay an additional $60/month for your portion of the 2026 medical cost. The deadline to participate is September 30, 2025. 


Where to Participate-

  • On-site screening locations will be offered at the following locations in Nebraska. Due to participation requirements we are not able to offer on site screenings in other locations. Select the location during the registration process. 

    • Axtell 7:30am-11:30am on September 16th

    • Beatrice 7:30am-11:30am on September 18th 

  • Your primary care physician. Download the form in the registration process. If you have already had your 2025 physical, you can register on the Quest site and download the physician form to have your physician complete with your information from your 2025 office visit. 

  • A Quest facility nearest to you. The facility nearest to you will be determined in the registration process. 


How To Get Started-

  • After you receive the email notification from Quest, Visit My.QuestForHealth.com

    • Returning Participants:

      • Log in with the username and password you created previously

      • If needed, use the username or password links to retrieve your login information

  • New Participants- In the Create Account area (green box), enter your Registration Key: mosaic2025

  • Click the Register Now button

  • When prompted, enter your Unique ID: Employee ID

    •     If your ID has leading zeros, please exclude the leading zeros

  • Under Wellness Screening, select an option to schedule your screening, this can be done by registering for a screening at a Quest Patient Service Center, by downloading a Physician’s Results Form for your provider to complete, or by registering at the Mosaic clinic in Axtell or Beatrice.
