There is no form.  You would give the doctor or optician's office your VSP card and they file the claim if they are in network.  We no longer have the $150 reimbursement plan since we have the stand alone vision insurance.  If you went out of network, you need to register at www.vsp.com and then log in and go to Your Benefit & Claim History and then click on How to Use My Benefits.  Once you are there you will see this and you can click on file a claim.
