To change your name you would follow Employee Self Service>Personal Details>Name.  Once this is done the status will change to pending approval.  Once approved, this will flow to Payroll for your paycheck and tax purposes.

If your name change is because of marriage/divorce you may want to complete a Qualifying Life Event.  To enter a Qualifying Life Event for Benefits you would follow Employee Self Service>Benefit Details>Life Event.


To change your name for your Mosaic email you would reach out to IT using the IT Portal https://mosaicinfo.freshservice.com/support/tickets/new or call 877-991-9628.
